Registration Materials:

All registration materials including a general information newsletter, the CBVC athlete information form, the CBVC Parent Volunteer Information Form,  the OVA registration form, and the Parent Code of Conduct are available for viewing under the Registration Forms tab.

Successful athletes will be given a registration package by their coaches. Successful athletes will be responsible for completing, printing and bringing ALL of these forms to the Team Registration Night on October 19th and 21st.

Please bring the following with you to registration:

Ballhawks Athlete Registration Form - ensure that your Health Card number and any health concerns are recorded on this form
OVA Registration form 
Ballhawks Parent Volunteer Information Form
Parent Code of Conduct - signed by parents or guardians
A copy of your birth certificate if you are new to the club

Code of Conduct for Parents Form - Please print, sign and hand in on your athlete's Registration Night
